What to eat on Ozempic: a practical Irish guide

7 min readDiet

On Ozempic, eat protein first, vegetables second, and everything else last. Appetite suppression means you will finish far less of a meal than you used to, so the order you eat in decides what you actually absorb. Aim for 25 to 40 g of protein per meal, three meals a day, alongside plenty of fluid. Eat slowly, stop when comfortable rather than full, and keep meals smaller and more frequent in the days after an injection when nausea peaks. There is no banned list, but very fatty, very large or very fast meals are what most people find they cannot tolerate.

Why the order you eat in matters more than usual

Semaglutide slows gastric emptying and raises satiety, which means you reach the point of comfortable fullness far earlier in a meal than you used to. Whatever is on the fork at that moment is what you have eaten.

If you start with potato and finish with the chicken, you will stop before the chicken. Do it for three months and you will have lost weight and a meaningful amount of muscle with it. Reversing the order costs nothing and changes the composition of what you lose.

How much protein, and why it is the whole game

Aim for 25 to 40 g per meal, which for most people lands at roughly 1.2 to 1.6 g per kilogram of body weight per day. This is the single most important dietary target on a GLP-1, and the one most people miss without noticing.

  • Two eggs: about 12 g. Not a protein meal on its own.
  • A chicken breast, roughly 150 g: about 35 g.
  • A tin of tuna or mackerel: about 25 g.
  • 170 g pot of natural or Greek yoghurt: about 15 to 18 g.
  • A tin of chickpeas: about 20 g, and it brings fibre with it.
  • A pint of milk: about 19 g, which is a genuinely useful option when eating feels like work.

What breakfast should look like

Breakfast is where the protein target is usually lost. Toast, cereal or a scone with tea is somewhere between 5 and 10 g, and if you have suppressed appetite you will not make it up later.

Eggs on brown bread, porridge made with milk and a spoon of nut butter, or natural yoghurt with berries all clear 20 g without much effort. If mornings are the worst time for nausea, which is common, a glass of milk or a small yoghurt is far better than skipping.

A realistic day of eating

Not a meal plan, just a shape that works and uses food from any Irish shop.

  • Breakfast. Two eggs on a slice of brown soda bread, tea. Around 20 g protein.
  • Lunch. Tin of mackerel or leftover chicken with a large salad and a small potato. Around 30 g.
  • Dinner. Fish or chicken with vegetables and rice, protein eaten first. Around 35 g.
  • If needed. A yoghurt or a glass of milk. Around 15 g.
  • Throughout. Water, and more of it than feels necessary.

That is roughly 100 g of protein without any powders, supplements or specialist shopping. If you are heavier than about 85 kg you will need to push higher, and that is when a protein drink starts to earn its place.

Fibre and fluid, the two things people forget

Constipation is one of the most common complaints on GLP-1 treatment, and it is largely a consequence of eating much less overall. Fibre and water fix most of it.

Vegetables, pulses, oats, wholegrain bread and fruit all help. So does drinking considerably more than you think you need, because reduced appetite tends to reduce thirst signalling alongside it. If constipation persists past a couple of weeks, mention it to your pharmacist rather than enduring it.

The days after an injection

Nausea clusters in the two or three days after your injection and after each dose increase. This is the window where what you eat matters most for how you feel.

Smaller and more frequent works better than three normal meals. Plainer food is easier than rich food. Eating slowly makes a genuine difference, because the fullness signal arrives later than the food does. Most people find that after two or three dose steps this settles considerably.

What about alcohol, coffee and takeaways

Nothing is forbidden, but three things behave differently on a GLP-1.

Alcohol largely bypasses the satiety mechanism, so it adds calories without producing fullness. Very fatty food, including most takeaways, sits in a slowed stomach and is the most common trigger for feeling genuinely unwell. And large meals, however healthy, are simply harder to tolerate than they were.

None of this needs a special diet or a health food shop. It needs protein first, meals a bit smaller, and more water.

When to get proper dietary advice

If you have type 2 diabetes, coeliac disease, kidney disease, a history of an eating disorder, or you are vegetarian or vegan and struggling to reach the protein target, this article is not enough. Ask your GP for a referral to a dietitian, or contact the Irish Nutrition and Dietetic Institute for a private practitioner. It is a short conversation that prevents months of guessing.

What to eat on Ozempic: common questions

Protein first, vegetables second, carbohydrates and fats last. Aim for 25 to 40 grams of protein per meal across three meals, plenty of fluid and adequate fibre. Because appetite suppression means you finish less of the plate, the order you eat in determines what you actually absorb.

Roughly 1.2 to 1.6 grams per kilogram of body weight per day, which works out at about 25 to 40 grams per meal. At 90 kg that is 108 to 144 grams daily. Falling short is the most common reason people lose muscle alongside fat.

Something that clears 20 grams of protein: eggs on brown bread, porridge made with milk, or natural yoghurt. Toast or cereal alone gives you 5 to 10 grams, and with suppressed appetite you will not make that up later in the day.

Smaller, plainer and more frequent meals work better than three normal ones. Soup, porridge, plain toast, yoghurt and milk are usually tolerated. Nausea clusters in the two or three days after an injection and after each dose increase, then settles.

Not usually. Around 100 grams a day is achievable from eggs, fish, chicken, dairy and pulses. Above roughly 85 kg body weight the target rises and a protein drink becomes a practical way to close the gap, particularly on days when eating feels like work.

No. Carbohydrates are not banned, they simply come last on the plate. The reason is that with a suppressed appetite you will stop eating before you finish, and you want the protein already eaten by that point.
