The Ozempic cost Ireland question has two answers and the gap between them is enormous. Reimbursed for type 2 diabetes, your cost is capped at €80 a month. Privately for weight loss it is €260 to €337 a pen, every month, with no State contribution.

Cost per kilogram lost: the figure that changes the ranking
Comparing pack prices tells you what you hand over at the counter. It does not tell you what the treatment costs to achieve a result, and on that measure the order changes completely.

Wegovy comes out cheapest per kilogram, not Saxenda. Saxenda has the lowest entry price of the five at €149.99, which is why it often looks like the budget option, but it is a daily injection at a lower average result, so the twelve-month figure lands near the top.
What a full year of treatment actually costs in Ireland
The pack price is roughly two thirds of what you will spend. A realistic first-year budget, privately and without any scheme cover:
- Medication, €1,800 to €6,700 depending on treatment and how long you spend titrating up.
- Initial consultation, €35 to €90. GPs sit at the lower end, private weight management clinics at the upper end.
- Baseline bloods, €60 to €100. Usually HbA1c, kidney and liver function, and thyroid.
- Follow-up reviews, €30 to €60 each. Expect four to six in the first year, more if you tolerate the dose poorly.
- Repeat prescriptions, €15 to €25 each where your prescriber charges for them.
- Sharps disposal, small but real, and higher on a daily treatment than a weekly one.
Add roughly €300 to €500 to any medication figure on this page to get a year-one total. From year two the consultation load drops, so the medication becomes almost the whole cost.
Why the same treatment costs different amounts at different pharmacies
Ireland does not fix the retail price of a prescription medicine that is not being reimbursed. A pharmacy sets its own dispensing fee and its own margin, so the identical pack can differ by €40 or more between two pharmacies in the same city. Nothing about the medicine differs.
Three things move the number: whether the pharmacy buys through a wholesaler or direct, what dispensing fee it applies per item, and whether it treats weight management as a service line worth competing on. Chains and independents both land at either end, so the name above the door predicts very little.

What moves these prices
Prices on this page change for four reasons, and it is worth knowing which is which before you assume a quote is wrong.
Manufacturer supply. Wegovy and Saxenda have both been through Europe-wide supply constraints. When supply tightens, low-cost pharmacies run out first and the remaining stock sits at higher prices.
Reimbursement decisions. If the HSE adds a treatment to the reimbursement list for weight management, the private price stops mattering for most people overnight. That has not happened for any weight loss indication in Ireland yet.
Strength changes during titration. Moving from a starting dose to a maintenance dose usually changes the pack you buy and therefore the price. Budgeting from the starting price understates the real cost.
Pack format. Multi-pen packs cost less per pen than singles. Saxenda drops from €149.99 for one pen to €106.00 per pen in a five-pack, which is a 29% difference for the same medicine.
