Which weight loss medications does the HSE actually pay for?
The HSE reimburses GLP-1 treatments when they are prescribed for type 2 diabetes, and does not reimburse any of them when prescribed for weight loss alone. That single rule, the indication rule, decides whether you pay €1.50, €80 or the full private price. No weight management indication is currently reimbursed in Ireland.
The indication rule that decides HSE reimbursement
Irish reimbursement does not attach to a medicine. It attaches to a medicine prescribed for a particular condition. The HSE reimburses semaglutide and tirzepatide for type 2 diabetes because that is the indication on the reimbursement list. The identical product prescribed for weight management is not on that list, so nothing is paid.
The practical consequence is stark. Two people can collect the same box from the same pharmacy on the same day, one paying €80 under the Drugs Payment Scheme and the other paying €554.95, and the difference is one line on the prescription.
The Drugs Payment Scheme and GLP-1 medication
The Drugs Payment Scheme caps what any Irish household pays for approved prescribed medicines at €80 a month. There is no means test and no age restriction. Anyone ordinarily resident in Ireland can register.
It only covers medicines reimbursed for the indication they were prescribed for. So it covers your Ozempic if that Ozempic is for type 2 diabetes. It does nothing at all for a GLP-1 prescribed for weight loss, and that is the point most people miss until they are at the counter.
The medical card
A medical card reduces prescribed medicines to a €1.50 charge per item, capped monthly. It is means tested.
Same rule applies. The card covers what is reimbursed for the indication written. It is not a general discount on anything a doctor writes down, and holding one does not make a weight loss prescription free.
The Long Term Illness Scheme
The Long Term Illness Scheme is the most generous of the three, and the narrowest. It provides medicines and appliances free of charge, with no means test and no cap, for sixteen listed conditions.
Diabetes mellitus is one of the sixteen. If you have type 2 diabetes and your GLP-1 is prescribed for it, the medicine is free. Obesity is not on the list, so weight management prescribing does not qualify.
Private health insurance
VHI, Laya and Irish Life Health generally do not cover the cost of GLP-1 medication. Outpatient benefit may contribute towards a consultation with a specialist, and some policies cover bariatric surgery under strict criteria, but the medicine itself is almost never paid for. What HSE reimbursement means for your monthly cost
- If you have type 2 diabetes, ask specifically whether a GLP-1 is appropriate for your diabetes management. If it is, your cost falls to €80, €1.50 or nothing depending on your scheme.
- If you do not, budget for the full private price and treat any scheme cover as a bonus rather than a plan.
- Either way, register for the Drugs Payment Scheme anyway. It is free, it takes minutes, and it covers your household’s other medicines.
Will the HSE reimburse weight loss medication in future?
It could. Reimbursement decisions are made by the HSE following a health technology assessment, and manufacturers have applied for weight management indications across Europe. If a weight management indication is added to the Irish reimbursement list, the private price stops mattering for most people almost overnight.

The scheme rules apply the same in every county
Worth stating plainly, because it is a common worry: the Drugs Payment Scheme, the medical card and the Long Term Illness Scheme are national. The €80 cap is the same in Donegal as it is in Dublin, and your pharmacy in Ennis applies the same rule as one in Rathmines.
What does vary is the private price you pay when no scheme applies, and how easy it is to reach a prescriber who will assess you. So if a scheme covers you, your county is irrelevant. If it does not, your county starts to matter quite a lot.
Does the HSE pay for weight loss medication: common questions
No. Ozempic is reimbursed for type 2 diabetes only. Prescribed for weight loss it is not reimbursed by any Irish scheme.
No. Wegovy is licensed for weight management, and no weight management indication is currently reimbursed in Ireland, so the €80 cap does not apply to it.
If you have type 2 diabetes and it is prescribed for that, yes, the Long Term Illness Scheme provides it free with no means test. Prescribed for weight loss, no.
Because Irish reimbursement follows the indication, not the product. The condition written on the prescription decides whether the State pays.
