How long does Mounjaro take to work?
Mounjaro reduces appetite within the first week, and most people see measurable weight change on the scale by weeks three to five — typically a little earlier than semaglutide. The full effect takes far longer. In SURMOUNT-1, participants on tirzepatide 15 mg lost an average of 20.9 per cent of body weight over 72 weeks, with roughly half of that reached by around week 24. The titration schedule means you spend your first month on 2.5 mg, a starting dose that is not expected to shift much weight.
What Mounjaro does in the first week
Tirzepatide, the active ingredient in Mounjaro, reaches peak blood concentration around 8 to 72 hours after injection and has a half-life of about five days. Like semaglutide it is a weekly injection, and like semaglutide the first thing you notice is appetite rather than weight.
What people report differently is intensity. Because tirzepatide acts on two receptors rather than one, the appetite change in the first week is often described as more abrupt than on Ozempic or Wegovy. Some find food simply stops being interesting. That is a normal response and not a sign the dose is too high.
Why Mounjaro works on two receptors, and why that matters for timing
Semaglutide is a GLP-1 receptor agonist. Tirzepatide is a dual agonist: it acts on GLP-1 and on GIP, a second gut hormone involved in insulin release and fat metabolism.
The practical consequence is a steeper curve. In SURPASS-2, a head-to-head trial in people with type 2 diabetes, tirzepatide outperformed semaglutide 1 mg on both weight and HbA1c at every dose tested. That does not mean Mounjaro is right for everyone, but it does mean the shape of the timeline is different, and you should not judge one drug by the other drug’s schedule.
When you will see weight change on the scale
Three to five weeks for most people, which is marginally earlier than the four to six weeks usually quoted for Ozempic. The amount at that point is still small: one to three kilograms is typical, and it is easy to mistake for normal fluctuation.
- Weeks 1 to 2. Appetite change, possible nausea. Scale usually flat.
- Weeks 3 to 5. First measurable weight change on 2.5 mg or shortly after the step to 5 mg.
- Weeks 8 to 24. The steepest part of the curve. Around half of total loss lands here.
- Weeks 24 to 72. Slower but continuing. This is where the difference from a crash diet shows.
- Beyond 72 weeks. Plateau, and the goal becomes maintenance.
What the SURMOUNT trials actually found
SURMOUNT-1 studied tirzepatide in adults with obesity or overweight without diabetes. Over 72 weeks, average weight reduction was about 15 per cent on 5 mg, 19.5 per cent on 10 mg and 20.9 per cent on 15 mg, against roughly 3 per cent on placebo.
Two things worth holding onto. First, these are averages across thousands of people, and the spread underneath them is wide in both directions. Second, every participant received lifestyle counselling alongside the drug. The trial did not test tirzepatide on its own, so neither should you read the numbers as what the injection does by itself.
Why it is slower for some people than for others
If you are eight weeks in and the scale has barely moved, one of these usually explains it.
- You are still low in the titration. Eight weeks means four weeks at 2.5 mg and four at 5 mg. The higher doses do the heavy lifting.
- Protein intake is too low. Appetite suppression makes it easy to undereat protein badly, and lean mass goes first.
- No resistance training. Without it, a larger share of the loss is muscle, which is the outcome you least want.
- Liquid calories. Tirzepatide slows gastric emptying, which does very little to a smoothie or a pint.
- Genuine low response. A minority of people respond poorly to tirzepatide. It is recognised, and it is not about willpower.
What to do if nothing has happened by week twelve
Twelve weeks at an adequate dose with no meaningful change is the point at which the manufacturer’s own guidance suggests reviewing whether to continue. Bring it to your prescriber rather than adjusting anything yourself. The usual options are a further dose increase, a check for an untreated cause such as thyroid dysfunction, or a change of treatment.
Do not increase your own dose, double up after a missed week, or buy higher-strength pens from an unlicensed seller. The HPRA has issued repeated warnings about falsified GLP-1 pens reaching Irish patients, and dosing errors with tirzepatide produce severe vomiting and dehydration.
Getting Mounjaro in Ireland
Mounjaro is prescription-only in Ireland. It is licensed both for type 2 diabetes and, separately, for weight management in adults meeting BMI criteria, which is a meaningful difference from Ozempic and affects both what your prescriber can write and what you pay.
If it is prescribed for type 2 diabetes, the Long Term Illness Scheme is worth checking before anything else, because it covers diabetes medicines with no income test. Otherwise the Drugs Payment Scheme caps a household at €80 a month across all prescribed medicines. For a private weight management prescription you pay the full cost, and you should budget for consultation and baseline bloods on top.
Supply has been intermittent across Irish pharmacies, more so outside Dublin and Cork. If your usual pharmacy in Galway, Limerick or Waterford is short, it is worth asking them to order rather than assuming the whole country is out.

Usually a substantial part of it. In the SURMOUNT-4 trial, participants who switched to placebo after 36 weeks regained around 14 per cent of body weight over the following year, while those who continued lost a further 5 per cent. Obesity is treated as a chronic condition for this reason.
Tirzepatide has a half-life of about five days, so it takes roughly three to four weeks after the final injection to clear substantially. Appetite typically returns before the drug has fully gone.
